Patient Information

When you come for your initial appointment, you will need to bring:
  • A referral from your Local Doctor/ General Practitioner or other medical specialist (doctor)*
  • All relevant scans and x-rays/test results
  • Relevant letters from your treating physiotherapist, chiropractor, allied health practitioner
  • A list of medications (if appropriate)
When you arrive for your appointment, you will be greeted by one of Dr Bloomfield’s secretaries, who will ask you to fill out an information sheet with important demographic information. Appointments are generally scheduled for 30 minutes and involve both discussing your problem (history of injury/illness) and an appropriate physical examination. Every effort is made to run to schedule but occasionally delays can occur due to the nature of medical consultations.

There is a charge for this consultation, part of which you can claim back from Medicare. This fee is payable at the time of consultation.

*Please note that from November 2010, Sport and Exercise Medicine (SEM) has been awarded specialist status by the Australian Government and Medicare and therefore a valid referral for your condition/injury from a medical practitioner (doctor) is required for your initial consultation in order for you to claim the maximum rebate for your consultation from Medicare. Please feel free to discuss this with the reception staff when booking your initial appointment.
